How To Nominate …

Please submit as much material as possible. Feel free to include copies of newspaper clippings, photos, letters of support from friends, family, coaches, etc., and extra sheets of paper if needed. Scrapbooks, if submitted, will be returned.

Below is a web form that you can use to submit a nomination. You can upload documents (.pdf, .doc, .jpg, etc.) using the web form. You are also welcome to complete the form online and then mail the printed material separately. If you wish to submit your ENTIRE nomination via the mail, print out this form.

Achievements
1) List the sport(s) that your nominee was able to attain outstanding achievement. Please indicate what level: high school, college, industrial, professional, etc., and the three years (example 1965-1998) of participation. The nomination committee is also interested in other sports your nominee participated in.
Honors
2) The listing of your nominee's specific accomplishment is the MOST important aspect of the nomination process. The more detailed information contained here, the more effectively the nomination committee will be able to consider your nominee. Please list in detail Special Honors & Awards such as All-County, All-State, # of Varsity Letters, Most Valuable Player, All-Pro, Statistics, etc.
Contributions
3) If your nominee is a contributor rather than a participant, list in detail the contributions made to Wayne County sports. This would include examples such as team sponsors, league officials, program founder, etc.
Comments
4) General Comments about your nominee in regards to accomplishments in the community, workplace, family, etc.
